Evolve ENT: Terms and Conditions

  • Consultation and procedural fees are payable on the day.
  • We accept EFTPOS or credit card payments only. We DO NOT accept cash or cheque.
  • Gap payments for surgery are payable 7 days prior to your theatre date. Please ensure you provide proof of payment if you elect to pay electronically or via online banking. Your theatre date will be cancelled and given to other patients if gap payments are not received on time.
  • By attending Evolve ENT and providing your information to our staff, you acknowledge that you have read and agree with our privacy policy.
  • We support a workplace free of bullying and harassment of any kind. If you cannot treat our staff with the respect and dignity they deserve you will be asked to leave. Any person wilfully damaging property, stealing or physically assaulting our staff will be reported to the police.
  • We appreciate that life happens and sometimes you won’t be able to make an appointment. We require 24 hours notice of all cancellations. Failure to do so will result in a cancellation fee. Further appointments will not be made if the cancellation fee is not paid prior.
  • We do not provide concession or pensioner discounts.
